Bluegrass PACE Care provides comprehensive care. From primary care to occupational therapy, exams and lab tests to medications, social activities to meals, all your health and personal care needs are centralized at PACE.
As a PACE participant, you live safely at home with support from a team of experts who meet with you regularly. Transportation is provided for care at the PACE Center, and you’ll never be rushed with your PACE team. Your care plan and schedule are created based on your needs.
What is PACE?
PACE stands for “Program of All-Inclusive Care for the Elderly.”
Bluegrass PACE Care keeps adults aged 55+, who need help with activities of daily living, such as meals, dressing, bathing or toileting, living safely at home.

- An all-inclusive care plan and all services to support that plan
- Transportation to and from the Bluegrass PACE Center and all authorized medical appointments

Who is Eligible for PACE Care?
To be eligible for Bluegrass PACE Care, the individual must:
- Be age 55 or older.
- Live in the service area of Fayette, Franklin, Anderson, Woodford, or Jessamine county.
- Need a specific level of care that may otherwise require a nursing home, such as assistance with meals, dressing, bathing or toileting.
- Be able to safely live in the community with help from PACE.

How is PACE Care Unique?
The healthcare team at Bluegrass PACE Care develops an individualized plan for each participant, with the goal of helping them maintain independence and safety while living at home. Our experts coordinate all aspects of the participant’s care and support, working with them to manage both everyday tasks and chronic medical conditions.
Care is provided at the Bluegrass PACE Center, with transportation to and from the Center and all authorized medical appointments. Meals and social activities, a full-service pharmacy, a gym for physical and occupational therapy, and other tools for excellent personal and medical care are all part of the program at the Bluegrass PACE Center.
A physician at Bluegrass PACE Care becomes the primary care doctor for all PACE participants. Participation in a PACE program means that the program provides all your health care. All medical care, including primary care and specialist services, are provided by the program or by an entity authorized by the program.
How is PACE Care Paid?
Services are 100 percent covered for participants with both Medicare and Medicaid, or Medicaid only. If you have only Medicare or private coverage, contact
Bluegrass PACE Care to discuss associated fees. Participants may pay privately for PACE services.
Because all healthcare services
must be received at the PACE Center or coordinated by the PACE program, participants may be fully and personally liable for the cost of services not authorized by PACE. Bluegrass Care Navigators can discuss eligibility and enrollment with you personally to best determine how we can support your needs.

A PACE participant may voluntarily disenroll from the program without cause at any time [42 CFR § 460.160 and 42 CFR § 460.162]. The disenrollment date will be coordinated between Medicare and Medicaid for a participant who is dually eligible.
