Welcome to our new website – and our new name!
We’re very excited about the changes, but rest assured, we are still the same great company, with the same caring people.
Since our founding as Community Hospice of Lexington, some 38 years ago, we have cared for thousands of families in our communities. However, we have grown to provide a wide range of services in addition to hospice care – including private nursing, case management, palliative care and grief care. We now provide expert care long before life’s final months.
So we added to our name, Hospice of the Bluegrass, the name Bluegrass Care Navigators to better represent all of our services. We still have the same compassion and commitment that has always defined our work.
“Care Navigators” puts us in a role of being knowledgeable and helpful professionals. When a patient or family member needs care, support and guidance for any serious, chronic or recurring illness, they don’t have to figure out which of our services is most appropriate. They need a guide, a navigator – someone to provide the information, assessment and resources to connect patients to the right care at the right time.
We also changed our service line names, to include Bluegrass Extra Care private duty and personal care; Bluegrass Transitional Care, with specially trained nurses to help after a hospital stay; Bluegrass Palliative Care; Bluegrass Hospice Care; and Bluegrass Grief Care.
Simply put, Bluegrass Care Navigators now has more ways to care.
