New Grants Expands Transportation Services for Older Adults in Bluegrass PACE Care

Bluegrass Care Navigators has received grant funding from Truist Foundation and the Lexington-Fayette Urban County Government (LFUCG) to purchase an additional transportation van for Bluegrass PACE participants in Fayette County. The new van will expand Bluegrass PACE Care’s existing transportation fleet, helping meet growing demand and ensuring participants can reliably access the care and services they need.

“We are incredibly grateful to Truist Foundation and LFUCG for their support,” said Jamie Moen, President of Bluegrass Care Navigators. “This additional van will help us reach more participants, reduce transportation barriers, and ensure people can consistently access the care, services, and social connections that help them live healthier, more independent lives.” 


In 2025, Bluegrass PACE Care provided comprehensive, all-inclusive care to more than 130 participants. The Program of All-Inclusive Care for the Elderly (PACE) serves adults age 55 and older who need support with activities of daily living and want to continue living safely at home. Bluegrass PACE Care offers a personalized care plan, primary and specialty medical care, medications, meals, and a dedicated healthcare team. 


“Several years ago, we started a grant program for non-profits that need to make capital improvements and operational investments – non-profits often find that grants they can qualify for cannot be used for these kinds of expenses,” Mayor Linda Gorton said. “The program has met many needs in the community, including this van for the Bluegrass Care Navigators’ PACE Center program.” 


In addition to medical and supportive services, Bluegrass PACE Care provides transportation to and from the PACE Center and to all authorized medical appointments. At the PACE Center, participants can enjoy meals, participate in group activities, and build meaningful relationships that support both physical health and emotional well-being.

With the addition of this van, Bluegrass Care Navigators will be better equipped to meet the growing transportation needs of Bluegrass PACE participants in Fayette County, helping more older adults stay connected to care and remain active in their community.
