Bluegrass PACE Care is all-inclusive healthcare that keeps Kentuckians 55+ living safely at home. Bluegrass PACE Care provides a healthcare team and a customized plan for your individual needs. PACE participants receive medical and personal care at the PACE Center, along with transportation, meals and social activities.
