Hazard Memorial Tree and Brick Pathway

Our Memorial Tree and Brick Tributes serve as peaceful reminders of the individuals that touch each of our lives. Located at the Greg and Noreen Wells Bluegrass Hospice Care Center, our brick and leaf tributes provide the community with an opportunity to reflect on their day or a loved one.

Tributes include small brick pavers, along with beautiful bronze leaves in varying sizes, along with animals, birds, and flowers for inscribing.

• $100 – For a gift of $100, you can have a 4”x8” Brick Paver in the Memorial Path. Line 1 can be “In Memory Of” or “In Honor Of”; line 2  can have a maximum of 14 characters including punctuation and spaces of inscribed type.
• $250 – For a gift of $250, you can have a small leaf on the Memorial Tree with two lines of 14 characters including punctuation and spaces of inscribed type.
• $500 – For a gift of $500, you can have a medium leaf on the Memorial Tree with three lines of 16 characters including punctuation and spaces of inscribed type
• $1,000 – For a gift of $1,000, you can have a large leaf on the Memorial Tree with four lines of 18 characters including punctuation and spaces of inscribed type.
• $2,500 – For a gift of $2,500, you can have a bird, animal, or flower on the Memorial Tree with four lines of 28 characters including punctuation and spaces of inscribed type.
