Bright Path Program Provides Hope to Chronically Ill Children and Families

Bright Path, a pediatric palliative care program, provides specialized medical support for medically fragile, chronically ill children and families.

Focusing on individual needs over prognosis, the program offers home-based services, tailored care coordination, symptom management, and shared decision-making. Each child receives personalized assistance, comfort and guidance through their unique journey, ensuring they and their families receive the support they need during this challenging time. 


"Hope is hard to describe sometimes," said Lauren Samuels, Bright Path LPN. "We have had a lot of success with this program, but it’s important to understand that success looks different for each family. Bright Path helps families find their unique path to hope.” 


The true magic of Bright Path lies in our presence in the homes of our patients. This allows us to be more than just health care providers; we become advocates with the families for services and connect them with the resources, equipment and treatments our youngest patients need to thrive. By improving communication between different specialists, we ensure that everyone is on the same page, creating a seamless web of support around each child and family.

Whether it's helping children access Medicaid waivers, overcoming insurance denials for crucial equipment or medications, securing waivers for families to provide care at home, or arranging daily nursing support, we're not just providing services; we're igniting hope. By connecting families with resources they couldn't pursue alone, we're making a tangible difference in their lives.” 


Samuels shares the story of Abbi*, now 14, who battles a rare genetic disorder that causes chronic migraines, among other symptoms that leaves her bedbound, with limited physical abilities and speech difficulties. Her mom serves as her primary caregiver, while also caring for Abbi’s younger siblings. Abbi is bedbound and prior to enrolling in Bright Path, her family struggled with inconsistent nursing support and only left home for necessary medical appointments. Since enrolling in Bright Path, the team helped Abbi’s family secure approval for home and community-based waivers that allow her mom to be compensated as a caregiver and receive the support of consistent nursing care. A nurse practitioner now effectively manages her chronic migraines, offering her an improved quality of life and peace of mind to her family. 


“Our goal is to improve the quality of life of for your family, by providing comfort to your child while building your confidence with education and support, and I love seeing the difference that we make in the lives of these children every day,” Samuels said. 


Bright Path can help manage symptoms for many pediatric illnesses and chronic conditions, including progressive cancer, neuromuscular disorders, genetic or congenital diseases, neurodegenerative disorders, cardiovascular conditions, pulmonary disorders and more.

Bluegrass Care Navigators has received grant funding from Truist Foundation and the Lexington-Fayette Urban County Government (LFUCG) to purchase an additional transportation van for Bluegrass PACE participants in Fayette County. The new van will expand Bluegrass PACE Care’s existing transportation fleet, helping meet growing demand and ensuring participants can reliably access the care and services they need.
